By providing us with your current insurance information, we will authorize your coverage benefits prior to your procedure and then bill your insurance carrier for our services directly after your procedure.
Please be aware of any admission policies your insurance plan may have. You or your physician may have to adhere to certain requirements in order to insure maximum reimbursement. Failure to obtain pre-authorization, physician referral or a second opinion may greatly reduce or eliminate your benefits.
You should receive an explanation of benefits from your insurance company showing how our claim was processed and indicating your responsibility for any applicable balance. You will also receive a statement from our facility reflecting any amounts paid and any discounts or adjustments to your account.

ALPHAEON CREDIT Card Accounts are offered by Comenity Capital Bank which determines qualifications for credit and promotion eligibility. Account must be in good standing at the time of purchase. At no time will the minimum payment due be less than $27.00. Minimum interest charge is $3.00 per credit plan. Standard variable APR of 32.99%, based on the Prime Rate.
