Insured Patients

As a courtesy, we will bill your primary and/or secondary insurance carrier or governmental agency directly for our facility fee (see SSC Facility Fee in this menu for more details).

By providing us with your current insurance information, we will authorize your coverage benefits prior to your procedure and then bill your insurance carrier for our services directly after your procedure.

Please be aware of any admission policies your insurance plan may have. You or your physician may have to adhere to certain requirements in order to insure maximum reimbursement. Failure to obtain pre-authorization, physician referral or a second opinion may greatly reduce or eliminate your benefits.

You should receive an explanation of benefits from your insurance company showing how our claim was processed and indicating your responsibility for any applicable balance. You will also receive a statement from our facility reflecting any amounts paid and any discounts or adjustments to your account.

SSC Facility Fee

The billing statement you will receive from Southgate Surgery Center is our facility fee for your procedure. Our fee includes:

• Patient facility use
• Nursing staff and technical personnel services in pre-op, OR and post-op
• Medications and supplies, including those used for anesthesia
• Surgical dressings, supplies and clinical equipment
• Lens implants (if applicable)
• Infection control items and services
• Administrative services and staff
• Scheduling and medical record supplies
• Technology (ORA, Laser Cataract Surgery)

Your surgeon and anesthesia team may bill you separately for their services. To learn more about other provider bills, click on each menu item to the left of this page.

Surgeon

Your performing physician will bill you separately for their surgical services. Contact your surgeon's office about anything related to their bill.


Anesthesia

Your anesthesia is administered by an anesthesia care team -- a certified registered nurse anesthetist (CRNA) and a physician anesthesiologist, who medically directs the CRNA. As such, depending on your insurance provider, you may receive two separate bills for the anesthesia services; one for the nurse anesthetist and one for the physician.

Note that the total amount is the same whether the anesthesia is administered by a care team or by a single provider.

SSC Payment Terms

All insured patient accounts with outstanding balances are due upon receipt of our statement. After 90 days, past-due accounts are submitted to a collection agency.

When making a payment, please provide us with your account number. It is a
six-digit number located near the upper-right corner of your billing statement.
